Denominator: All patients evaluated by the Eligible Professional in Emergency Services, Inpatient, or Post-Acute Care setting (E/M Codes 99221-99223, 99231-99233, 99234-99236, 99238-99239, 99291-99292, 99304-99310, 99315, 99316 AND Place of Service indicators 02, 10, 19, 21, 22, 23, 31 or 32 OR equivalent in standardized code sets) AND
• Diagnosis of Advanced Illness
NOTE: This measure is to be submitted a minimum of once per hospitalization for patients seen during the performance period.
